Join us for an unforgettable day as we spend the day at Wild Adventures and then head to the For King & Country concert that evening! It's going to be an incredible day of fun, fellowship, and worship. You won't want to miss it!

ANNUAL YOUTH MINISTRY PERMISSION SLIP
2026 - 2027

Scope of Activities: The type of activities planned for this year are represented by the Youth Ministry Activities from previous years and ideas for the future (listed below). Your child will receive a detailed calendar prior to the events as specific activities are scheduled. Please call the Youth Leaders if there is any question about a specific activity that is scheduled. This Permission Form covers all activities of durations up to and including week long outings. International trips will require and additional permission slip.
Transportation: Transportation will be provided for all Youth Activities once your child arrives at Church. In most cases, transportation will be provided via church owned vehicles or rented vehicles.
  • Adult Youth Workers may transport teens in their personal vehicles and be the only adult in the vehicle.
  • On very rare occasions, teens may be permitted to ride with other teens, but only with verbal consent from a parent or guardian.
Overnight Accommodations: The following will be adhered to during any overnight activity:
  • Teens and Youth Workers will always be separated by gender for sleeping arrangements
  • No one will be permitted in a room reserved for the other gender
  • An individual Adult Worker will never be alone in a room with only one teen
  • One Adult Worker may be sleeping in a room with two or more teens
  • On rare occasions two or more teens may be sleeping in a room where the Adult Worker is in the room next door

I hereby give permission for my child to participate in all of the activities of the University Church Youth Ministry (unless otherwise indicated) for the 2025-2026 Calendar Year as typically represented with the activities listed below. I hereby acknowledge and accept the risks of physical injury associated with participation in the activities of this ministry. Except for gross negligence on the part of the University Church or its representatives – I accept personal financial responsibility for any personal injury sustained during these activities and I promise to hold University Church and its representatives harmless for any injury related to the activities.

If you have any questions about the details surrounding a particular outing - please call one of the Youth Workers and ask. Many outings have gym and camp type activities. Some outings are chaperoned “overnighters”. Most outings include some kind of a Bible message.
